Welcome to the Registration form for "Super Mom 2009"

Before you complete the form, please read the following instructions :

CONTEST ELIGIBILITY:

To enter the SUPER MOM 2009 contest you must be a US resident. You must also be the mother of at least one child.All family and friends are welcome to watch & cheer the contestants and enjoy the various activities, food and entertainment.

ACTIVITIES FOR THE FAMILY:

The participants can take part in any of the following activities, if they choose, from 4pm-9pm...the idea is to enjoy working out your body,mind & intellect!!

Round 1: Fitness
All contestants will choose one of the following three activities to participate in for this round:
5K FUN RUN/WALK
FUN RELAY RACE
HULA HOOPS
30 contestants will advance to the second round.

Round 2: Talent
30 winners from the fitness round will choose one of the following three activities to participate in for this round:(required material will be provided at the venue)
ARTS
CRAFTS
PERFORMING ARTS (MUSIC/DANCE/ACTING ETC.)
10 finalists will advance to the final round.

Round 3: Knowledge and Personality
The 10 finalists will be asked questions that will test her GENERAL KNOWLEDGE and PERSONALITY. Based on her answers, judges will declare SUPER MOM 2009 and 2 RUNNERS-UP.

Apart from this, the family can enjoy various activities including TRICKY TRAY, FASHION SHOW, LIVE MUSIC, CHILDRENs GAMES, and FOOD etc. at the event.

ENTRY FEE:

Moms: $25.00 to enter (every participant gets a Gift Bag with giveaways worth $40);Mom's family can come FREE which includes 1 Adult who accompanies the Mom and all Children under 18 years

Non-contestants/visitors: $10.00


Price of ticket includes:5 TRICKY TRAY TICKETS, CHILDREN ACTIVITIES LIKE FACE PAINTING, DANCE/MARTIAL ARTS DEMO, LIVE MUSIC, and FASHION SHOW
